Myriad Morcells™ and Myriad Matrix™ Use in an Abdominal Trauma

Patient Presentation
- 51-Year-old male.
- The patient sustained a full-thickness abdominal trauma due to a high velocity motor vehicle accident. Following surgery, the wound dehisced resulting in a 25cm x 4cm defect with significant loss of domain and which was not able to be surgically closed (Figure 1).
Procedure and Treatment
- The patient was admitted to the operating room for emergent laparotomy, diaphragm and bladder repairs, and a bowel resection.
- Myriad Morcells (1000 mg) was applied to the open area of the defect (Figure 2) followed by Myriad Matrix (3-layer, 10 x 20 cm) to bolster the Myriad Morcells (Figure 3). The defect was then covered by Adaptic™ and NPWT.
Clinical Outcomes
- Day 6. The Myriad products were well adhered with buds of granulation tissue observed as they integrated (Figure 4).
- Day 18. Significant granulation tissue observed with residual Myriad present on the defect surface (Figure 5)
- Week 5. Myriad had completely integrated, and granulation tissue had significantly filled the depth of the defect. Endoform Natural™ was applied to drive closure and prepare the surface for a split-thickness skin graft (STSG) (Figure 6)
- Week 6. Complete contour restoration achieved enabling the planned STSG to be placed (Figure 7).
- 4 months. The STSG had fully epithelialized, and the defect had closed. There were no complications or recurrence (Figure 8)
