Stefan Holubar, MD, on Perioperative Management of IBD

Dr Holubar reviewed the key steps to preparing patients with inflammatory bowel disease for surgery to ensure the best possible outcomes at the Advances in Inflammatory Bowel Diseases virtual regional meeting April 20-21.

Stefan Holubar, MD, is the IBD surgery section chief and a director of research for the Department of Colorectal Surgery at Cleveland Clinic in Cleveland, Ohio.

Hi, I'm Stefan Holubar. I'm the IBD Surgery section chief and a director of research for the Department of Colorectal Surgery at Cleveland Clinic. And I presented the perioperative management of inflammatory bowel disease at AIBD Regionals. And first, we reviewed the goals of surgery, which are very different for ulcerative colitis and Crohn's disease. With Crohn's disease, we focus on treating the complications of the Crohn's disease, because we can't cure Crohn's disease with surgery, nd the fact that surgery is curative for the colon and rectal manifestations of ulcerative colitis.

Both surgeries are aimed at providing the patients with the best quality of life that we can. Then, we went on to some nitty-gritty details regarding all the different types of surgery for IBD, except perianal, which is covered in a separate talk. We reviewed bowel-preserving surgery with strictureplasties and segmental small bowel resections for Crohn's disease. We into some depth on ileocolic Crohn's disease, including the Kono-S and mesenteric excision, both which are hot topics right now. And then, everything you need to know in brief about total proctocolectomy with either a pouch, and the ileostomy for ulcerative colitis.
