Adventist HealthCare Selects Wellcentive to Advance Population Health Management throughout its Clinically Integrated System

ATLANTA -- Wellcentive, a leader in value-based care solutions, has announced a comprehensive agreement providing population health management technology to Adventist HealthCare.

Adventist HealthCare is a faith-based, not-for-profit, integrated healthcare delivery system based in Gaithersburg, MD that includes nationally accredited acute care and specialty hospitals, mental health services and home health agencies serving the Washington, D.C., metropolitan area.

In addition, Adventist manages the Mid-Atlantic Primary Care ACO, one of the largest Medicare Shared Savings Programs serving 14,000 Medicare patients through its physician network of more than 1,000 providers in the metropolitan D.C. area. The Adventist physician team also supports the One Health Quality Alliance (OHQA), a physician-led, clinically integrated network of more than 40 physician groups.

Integrating Social Determinants of Health

As part of the partnership's approach to patient care, Wellcentive will develop the ability to integrate privacy-sensitive aspects of social determinants of health into patient data, sharable throughout care teams as part of data aggregation and translation.

"For our patient population, and really for every patient population in America, understanding the factors that impact health beyond episodic visits is a critical piece of population health management and comprehensive care," said Patrick Garrett, M.D., senior vice president, physician strategy, Adventist HealthCare.

"We have defined a strategic roadmap to provide high-quality, coordinated care through our physician networks that include the ACO as well as through the physician partnerships in the One Health Quality Alliance. Deploying Wellcentive's data integration solutions will allow us to strengthen our independent physician, acute and post-acute providers' ability to deliver truly coordinated care," added Garrett.

The World Health Organization (WHO) and the Centers for Disease Control and Prevention (CDC) have long recognized that social determinants of health impact care plan adherence, health risk factors and the success of chronic care and outcomes.

Social determinants of health range from housing, literacy, job training and access to transportation and social support networks to socioeconomic levels, exposure to crime and violence, access to media and even cultural norms.

In addition to this data build, Wellcentive will also integrate Hierarchical Condition Category (HCC) codes as part of the Centers for Medicare & Medicaid Services (CMS) risk adjustment model framed to chronic care.
