Impasse Sign in Balloon Mitral Valvotomy

Video. During balloon inflation in the left ventricle, the proximal balloon inflated before the distal end; after seeing it, the balloon was deflated immediately. This is suggestive of the ‘impasse sign’ due to entanglement of the distal balloon in the chordal apparatus. Full balloon inflation in such scenarios can lead to severe mitral regurgitation.
