Instruct patients/nurses to remove the compression system if the foot or toes become pale or discolored, painful, numb/tingling or cool to the touch.
These signs may suggest that arterial blood flow is compromised by the compression device.
DeRoyal Unna Boot should always be applied by a trained health care provider. Incorrect application may result in inappropriate levels of compression. Low levels of compression will not provide adequate level of compression to achieve desired compression effects and limb stability, whereas high levels of compression may interfere with arterial blood flow to the distal extremity.

Wash intact skin and pat dry. Rinse any open wound(s) with sterile normal saline. May treat wound(s) with Multidex® powder or gel prior to applying the Unna Boot.
Position foot in a 90° angle and maintain position throughout application. Begin on top of the foot at the base of the toes. Loosely apply the dressing using the spiral wrapping technique with 50% overlap. Avoid creasing and folding. Cut any excess dressing to avoid overlapping below the knee. Apply an elastic or cohesive bandage to secure dressing and maintain compression.
